First and foremost, we are excited to welcome you to the Duluth Family! We offer a convenient, online registration process where supporting documents are uploaded while completing the online registration.

Please note, once you submit your online registration, you will no longer be able to upload documents.

When we receive the registration and the supporting documentation, you will receive an email with your appointment information.
